Two days prior to a routine appointment, our automated system will call or text with a reminder of your appointment.

You must notify our office at least 24 HOURS in ADVANCE if you are unable to keep your appointment. Failure to do so will result in a $50 surcharge being placed on your account.

New Patients that do not keep their appointment will not be permitted to return to our office.

What to Bring:

Insurance card(s) and Identification (Driver's License) Medicare card, plus the date of your spouse's retirement (if applicable)

List of current medications and dosages (preferably, bring your bottles with you)

Any pertinent medical records, lab or x-ray results. 
Your co-pay (the office accepts cash, checks, VISA and MasterCard)

Your employer's name, address and phone number - if you are covered by traditional insurance or managed care (HMO or PPO) provided by that employer.

Your spouse's employer's name, address and phone number if your are covered by traditional insurance or managed care (HMO or PPO) providee by your spouse's employer
